Vaccinations for Travelling Abroad

If you are travelling abroad and require vaccinations or medication, you will need to complete the 'Walderslade Surgery Foreign Travel Risk Assessment Form' before making an appointment to see the nurse for advice about your vaccinations. Download the form here:

Due to a UK-wide shortage of some of the recommended vaccines for travel, the Practice is only able to offer a limited service.

Travellers are advised to plan well in advance and to seek travel health advice from a suitably qualified healthcare professional 6-8 weeks prior to their trip.
